Between conventional and unconventional, there is currently at least one hybrid - the EzGrip, a relatively ordinary looking ballpoint with a "saddle" or a "support ledge" on the front.
The "support ledge" makes it possible for the writer to use only a single fingertip, knuckle or thumb to press pen to paper and guide it - no need to squeeze the pen's sides.
This is a bonus for those who find pens hard to grip. The unique "saddle" has been reproduced as a rubber slip-on and is of enormous benefit for students and those who write prolifically as well as those who need to grip paint brushes and small tools.
